Questionnaire on current commissioning and contracting arrangements for patient transport

This questionnaire is for a senior manager responsible for commissioning cancer patient transport to complete. We are asking about commissioning and contract arrangements for patient transport in the catchment area for Mount Vernon Cancer Centre. The answers to this questionnaire will be used to influence the Mount Vernon Cancer Centre Review and look at the role of patient transport in current and future services.
